The Community Diabetes Education Center is pleased to offer diabetes on demand services to existing patients. On demand service is a partnership between patients and our center to aid in providing education and support when you need it most and at your convenience.
To access Diabetes on Demand, follow these three easy steps:
For existing patients that have previously been seen, if you have a question or concern about anything related to your diabetes, diabetes medication, blood sugars, diet, etc., contact our office to set up Diabetes on Demand services.
Once you contact our office, we will set you up with a personal virtual session with one of our diabetes care and education specialists within two hours from the time of your call (some restrictions may apply).
For Diabetes on Demand requests that are received after 3:00 p.m., or outside of normal business hours, your session may be scheduled for the next business day.
At the time of your session, simply log in to MyHealthMate and follow the prompts to begin the visit. If you do not have a MyHealthMate account, we will send you a text message with a link to join the session.
